  • Consent for Medical
  • Food Program Application
  • Child/Infant Form
  • Student Information Form
  • Parent Handbook Signature Page
  • Physician Report (Due within 30 days of initial start date) **


** If your child has food allergies, it should be noted on the physician’s report. Once noted by doctor, the preschool will supplement child’s meals with an alternative food. Any food preferences, parents will need to provide the alternative food.


Items Needed for Classroom


For All Ages:

  • Complete set of labeled extra clothes in large ziplock bag
  • Crib size sheet & blanket if napping
  • Earthquake kit


For Infants Only:

  • Diapers
  • 2 sets of clothes
  • Crib size sheet & blanket
  • Jarred baby food
  • Bottles
  • Formula (if you choose to opt out of school supplied formula)
